Dear Parents/ Carers
RE: Health Care Plans 2025/2026
In order to ensure that we have the full, correct information to support any medical needs that your child may have when attending Sir Geoffrey Leigh Academy, we would ask you to kindly complete the Health Care Plan if your child has a new or existing medical condition
A Health Care Plan will ensure that your child is cared for appropriately and that members of staff are aware of important information such as symptoms, treatment, medication and correct dosage of medication. It is also important that we have a written record of what you would like us to do in the event of an emergency due to your child’s health needs.
If your child requires medication at the academy, please ensure that it is present and in date and in the original packaging. An additional form will need to be completed to give permission to administer medication. We will contact you at the end of each module to update your child’s health care plan to ensure that we have the most up to date records. If there is an update mid-module please contact your college admin team to update us.

The college administrative team will update the system to say that the module’s review has been completed and will update the medical plan on record. Please return to SGLA by Friday 14th November 2025.
Please note, if we do not receive a response from you – we will work on the basis that no update has been provided and the health care plan that is currently in place is the correct provision.
